We are thankful for the collaboration of Shelter Portland in opening this day program to address social determinants of health in East Portland. We are also grateful for Trillium Community Health Plan for have been awarded the 2023 Community Benefit Initiatives Grant that will sustain much of the programming this year, as well as the ability to purchase two vehicles that will support direct outreach and community health work for vulnerable communities.
