- Monday–Thursday: 8:30 AM – 4:00 PM
- Friday: 11:30 AM – 7:00 PM
This position requires flexibility to work occasional evenings, weekends, and weather emergency activations with little notice. Participation in severe weather sheltering and emergency response efforts is required.

Position Summary
The Outreach Case Manager / Community Health Worker provides street outreach, resource navigation, shelter and housing support, and intensive case management services to individuals experiencing homelessness. This position serves as a bridge between vulnerable community members and systems of care, helping participants access shelter, housing opportunities, healthcare, identification documents, benefits, treatment services, and other critical resources.
This role combines direct field outreach with documentation, data entry, participant engagement, and ongoing case management. The ideal candidate is compassionate, adaptable, highly organized, and comfortable working in a fast-paced environment with individuals experiencing complex barriers.
Essential Duties and Responsibilities
Outreach and Engagement
- Conduct street outreach throughout Multnomah County, including encampments, public spaces, shelters, and community locations.
- Build trusting relationships with individuals experiencing homelessness.
- Provide resource navigation, referrals, and advocacy.
- Participate in coordinated outreach efforts with community partners.
- Respond to weather emergencies and community crises as needed.
Case Management and Navigation
- Complete participant intakes, assessments, and individualized service plans.
- Assist participants with shelter placement and housing navigation.
- Support access to identification documents, benefits, healthcare, behavioral health services, treatment, and employment resources.
- Coordinate services with community providers and partner organizations.
- Maintain regular participant contact and follow-up.
Documentation and Data Management
- Maintain accurate and timely participant records.
- Complete required data entry and documentation within designated databases and systems.
- Track outcomes, referrals, services provided, and participant progress.
- Ensure compliance with agency policies, confidentiality requirements, and contract standards.
Community Health Worker Functions
- Provide education regarding healthcare access, benefits, and community resources.
- Assist participants in navigating complex systems.
- Advocate for participant needs across healthcare, housing, and social service systems.
- Promote participant self-determination and empowerment.
Team Participation
- Attend staff meetings, trainings, supervision, and professional development opportunities.
- Participate in organizational events and community partnerships.
- Support agency operations during severe weather events and emergency activations.
- Uphold the mission, values, and culture of PDX Saints Love.
Required Qualifications
- Minimum one year of experience in outreach, case management, housing navigation, social services, behavioral health, recovery support, peer support, or related fields.
- Demonstrated experience working with individuals experiencing homelessness.
- Strong understanding of trauma-informed, person-centered, and harm reduction approaches.
- Experience with housing and shelter navigation.
- Strong computer skills, including data entry, documentation, and database management.
- Excellent communication and relationship-building skills.
- Ability to work independently and as part of a team.
- Valid driver's license, reliable personal vehicle, and proof of insurance.
- Ability to pass any contractually required background screenings.
Physical Requirements
- Ability to walk long distances in varying weather conditions.
- Ability to navigate uneven terrain and outdoor environments.
- Ability to lift and carry 40 pounds or more.
- Ability to sit, stand, bend, and move throughout the workday.
- Ability to safely transport outreach supplies and materials.
Preferred Qualifications
- Community Health Worker, Peer Support Specialist, QMHA, or related certification.
- Experience with HMIS or comparable data systems.
- Knowledge of Multnomah County homeless services systems and community resources.
- Bilingual or multilingual applicants are encouraged to apply.
- Lived experience of homelessness, recovery, behavioral health challenges, or system involvement is valued.
